False PHD is a passive item introduced in the Repentance DLC expansion for The Binding of Isaac: Rebirth. It functions as a corrupted counterpart to the standard PHD item, rewarding players for taking risky or negative pills. Core Effects
Pill Identification: Instantly identifies all pills in the current run upon pickup.
Item Spawn & Health: Spawns a random pill on the floor and grants Isaac one Black Heart upon pickup.
Pill Conversion: Automatically converts all “good” pills in the game’s rotation into their corresponding “bad” variants.
Stat Down Compensation: Whenever Isaac consumes a stat-decreasing pill, he gains a permanent +0.6 Damage upgrade (+1.2 for Horse Pills). This effect works retroactively for any stat-down pills taken earlier in the run.
Black Heart Generation: Consuming a non-stat-reducing bad pill (such as Amnesia, Addicted, or Paralysis) causes a Black Heart to drop on the ground. Important Item Interactions
PHD / Lucky Foot / Virgo: Having any of these items alongside False PHD allows pills to appear in both good and bad variations again. However, consuming any remaining bad pills will still trigger the False PHD damage boost or Black Heart reward.
A Pony / White Pony: Holding either item locks Isaac’s minimum movement speed to 1.50, effectively converting Speed Down pills into pure damage upgrades with zero movement penalty.
Placebo: When paired with a stat-down pill, it can be repeatedly activated to aggressively trade minor stats for massive, permanent damage scaling.
